Parties who cannot reach agreement about whom to appoint as a conciliator may apply to the CCG
to make an appointment on their behalf.

Please send the application, together with a cheque for £59.00 payable to CCG to
the address shown in the form. CCG will enquire as to the availability of
appropriate Conciliators and then make an appointment.
